Alternative Homes for Youth Community Transition Program

AHFY offers a range of services for youth preparing to reenter the community. Services ranging from GPS monitoring, obtaining vital records and job skills, to providing therapy services and facilitating restorative community justice, increase the likelihood youth will be successful. Our program ensures youth maintain a schedule that keeps them moving forward and holds them accountable to Multi-Disciplinary Team (MDT) transitional requirements.

Our Community Transition specialists will meet with the youth and his MDT prior to discharge. Recommendations from staffings, parole hearings, and a transition assessment will provide additional information we will use to generate a comprehensive picture of client needs. Then, in consultation with the youth’s MDT, we will create an individualized transition plan. 

Community Transition Services

The packages we offer can include the following services: 

• Job searching, creating a resume, completing job applications, and mock interviews
• Transportation, including teaching how to access and utilize public transportation 
• Accessing community resources, including food stamps, housing assistance, and mental health services
• School enrollment and applying for financial aid
• Obtaining vital records such as driver’s licenses, social security cards, and birth certificates
• Accessing local cultural and religious support
• Finding appropriate recreational and pro-social activities
• Offense specific community registration (as needed)
• Support in attending 12-step and similar groups
• Crisis intervention (as needed) via emergency pager
• Attending MDT staffings when appropriate
• Facilitating community service hours
• Support during court appearances  
• Independent living skills support (i.e. budgeting, shopping, home maintenance)

Community Transition Packages 
The following service packages are available. Please note that GPS monitoring is not included but is available on a fee for service basis: 

  • Community Transition – Low     
4-8 hours of transition services per month

  • Community Transition – Medium     
8-15 hours of transition services per month

  • Community Transition – High     
15+ hours of transition services per month
Emergency pager/on-call crisis management support included 

Community Transition with Therapy
  • Community Transition and Therapy – Low   
4-8 hours of transition services per month plus up to 3 hours/week therapy 

  • Community Transition and Therapy – Medium   
8-15 hours of transition services per month plus up to 3 hours/week therapy

  • Community Transition and Therapy – High   
15+ hours of transition services per month plus up to 3 hours/week therapy. Emergency pager/on-call crisis management support included

Services offered on a fee for services basis:
• GPS monitoring services  
• Restorative justice practices such as facilitating reintegration circles, victim empathy, and victim-offender conferencing  
• Drug and alcohol screening
• Spice screening 
• Emergency pager for low and medium transition services including response to personal crises with youth and GPS violations.
