Alternative Homes for Youth Community Transition Program

Alternative Homes for Youth (AHFY) offers a range of services for preparing youth to reenter the community. From help with obtaining vital records and finding employment and housing to providing counseling services we increase the likelihood of success. Our program ensures that youth maintain an active schedule that keeps them moving forward and holds them accountable to parole requirements. This program is accredited through COA.

Our Community Transition Specialists will meet with the youth and his Multi-Disciplinary Team (MDT) prior to parole. The Specialist will also attend staffings, parole hearings, and conduct a “transition assessment” to generate a comprehensive picture of the youth’s needs. In consultation with the MDT, an individualized transition plan for the youth will be created.

Activities Include:

  • Job searching, creating a resume, completing job applications, and mock interviews
  • Transportation; including teaching how to access and utilize public transportation
  • Accessing community resources such as food stamps, housing assistance, and counseling services
  • School enrollment and applying for financial aid
  • Obtaining vital records such as driver’s licenses, social security cards, and birth certificates
  • Finding appropriate recreational and pro-social activities
  • Accessing local cultural and religious support
  • Offense specific community registration
  • Support in attending 12-step and similar groups
  • Crisis intervention
  • Facilitating community service hours
  • Support during court appearances
  • Assistance locating and applying for housing in the Northern Colorado area
  • Other independent living skills

Community Transition Packages

The following service packages are available.

  • Community Transition – Low
    4-8 hours of transition services per month
  • Community Transition – Medium
    8-15 hours of transition services per month
  • Community Transition – High
    15+ hours of transition services per month
    Emergency pager/on-call crisis management support included

Community Transition with Therapy

  • Community Transition and Therapy – Low
    4-8 hours of transition services per month plus up to 3 hours/week therapy
  • Community Transition and Therapy – Medium
    8-15 hours of transition services per month plus up to 3 hours/week therapy
  • Community Transition and Therapy – High
    15+ hours of transition services per month plus up to 3 hours/week therapy. Emergency pager/on-call crisis management support included
